Faculty members may e-mail the department and have other persons who have University of Iowa library privileges to be listed as proxies on their own account. The Libraries (with the exception of Law) no longer give out proxy cards.  The request must come from the email address of the person requesting that someone be added as a proxy.  There is no fixed date of expiry--if they wish to end the individual's proxy term, they will have to specifically request this be done also by email. Proxy requests will show as if items were checked out on the parent record and there is no mention of the person checking out.  If the proxy wishes to check out a book or media item as a proxy they present their own ID card and the "use proxy" checkbox is activated. Check the faculty member's identity the user instructs the LA.  With the advent of office and home delivery, the use or proxies has become increasingly rare.

The form, if we continue to use it at all should be changed as there is no proxy card to pick up now and there is no expiration. The proxy service expires when the faculty member contacts us to tell us it has expired and we remove the name.
